Cass Health is seeking a Health Coach (RN) to join our team and help advance our mission of improving the health of the communities we serve. This position plays a key role in supporting patients across the care continuum by addressing preventive care needs, chronic disease management, and closing care gaps through data-driven outreach and patient engagement. Position Summary The Health Coach works collaboratively with patients, providers, clinical staff, and external partners to promote preventative care, chronic disease management, and improved health outcomes. This position serves as a liaison between patients, primary care providers, specialists, and community resources to support individual care plans, address barriers to care, and enhance communication across all levels of the healthcare system. The Health Coach will support both inpatient and outpatient services. Essential Responsibilities Serve as a care liaison between patients, providers, clinical teams, pharmacies, and specialists. Guide patients through medical therapies and care plans using clinician-approved protocols, escalating concerns as needed. Engage with patients and caregivers to increase activation, address barriers, and support healthcare goals. Conduct wellness programming in areas such as nutrition, weight management, tobacco cessation, physical activity, and preventive screenings. Collaborate with third-party healthcare providers and wellness vendors for appropriate referrals as directed by providers or evidence-based protocols. Participate in quality improvement activities to support clinic- and organization-wide performance goals. Establish rapport with patients via in-person, telephonic, and electronic communications. Utilize the electronic health record (EHR) for documentation, reporting, and population health management. Support ambulatory clinic nursing as needed. Perform other duties as assigned. Education & Qualifications Current Iowa Registered Nursing License (or valid license from a compact state) BLS certification (within 6 months of hire) Valid Driver's License Obtain Health Coach Certification within 1 year of employment Minimum of 2 years nursing experience Previous experience in care coordination, population health, or chronic disease management preferred.
